An electric multiple unit crashed into another near Kalafong station on the morning of Thursday 31 January 2013, both packed with school children and rush-hour commuters.  Up to 300 people were injured, 28 seriously, including the second train's driver. Authorities blame the crash on the theft of 25 metres of copper cable linked to the signal system.  The multiple units in service are up to 50 years old with the most recent from 1986. (thanks to Wallace for sending these in)
